How to Report a Claim
A letter and copies of the following information should be sent by mail (regular or overnight), fax, or by e-mail:
- Bond number
- Claimant contact information (name, address, phone, fax)
- Description of loss (including date of occurrence and amount of loss incurred)
- Documentation supporting loss
- Documentation of lawsuit, service of same (if applicable)
- Any additional information on the principal (location at time of loss, phone numbers, addresses, etc.)
